Born in Brazil: urbanisation and a free health system

Life expectancy in Brazil was around 55 in the 1960s and is 76 today. The number of children per woman fell from about six to 1.6 over the same period, dropping below the replacement level in the mid-2000s. The country urbanised very quickly, and 88 per cent of the population now lives in cities. The free public health system set up in 1988 spread vaccination and infant health services.

The differences within the country are still large. Income and services are high in the southeast, around São Paulo and Rio, and lower in the northeast and the Amazon region. Even within a single city, life expectancy can differ by years between a wealthy district and a poor outlying one. Violence is also one of the factors pulling the average down, especially among young men. A Brazilian baby's chances are often decided by the region and neighbourhood where it is born and by its mother's education. Urbanisation and the growing share of women in education and the workforce account for much of the fall in fertility.
