Born in Pakistan: high fertility, uneven services

Life expectancy in Pakistan was around 45 in the 1960s and is 67 today. Vaccination and basic health services have spread, yet the fight against diseases such as polio continues. Pakistan has one of the highest fertility rates in South Asia, at 3.5 children per woman. India and Bangladesh have already dropped below the replacement level, while the decline in Pakistan has been slower.

Where you are born decides a lot. Health and school facilities are broader in Punjab and in big cities such as Karachi, and far more limited in the sparsely populated parts of Balochistan and in remote villages. School attendance among girls stays low in the countryside, and a mother's education is directly linked to both infant deaths and the number of children. Floods in some years also badly disrupt infrastructure and health services. A large number of children in a household divides the family income, which shrinks the share of schooling and care each child gets.
